Documentation is more than paperwork—it is an essential part of ethical, legal, and clinically responsible practice and not doing it correctly within guidelines may open you to legal action. This presentation explores practical strategies for creating clear, accurate, clinically relevant, and defensible documentation while maintaining client dignity and confidentiality. Participants will learn how to integrate clinical judgment, ethical standards, risk management, and documentation requirements into everyday practice.

Learning Objectives  

Objective 1: Identify the ethical, legal, and clinical purposes of professional documentation in mental health practice.

Objective 2: Differentiate between essential clinical information and unnecessary or potentially harmful details when documenting client care.

Objective 3: Apply ethical documentation principles to progress notes, treatment plans, assessments, and other clinical records.

Objective 4: Develop documentation practices that support continuity of care, protect client confidentiality, and demonstrate sound clinical decision-making.

Dr. Christie Rogers-Larke, EdD, LPCS

Outside of the Box Therapy

Dr. Christie Rogers-Larke is a licensed professional counselor with nearly 30 years of experience in the mental health field. She is a certified teacher and school counselor, adjunct professor, and Clinical Director of Outside of the Box Therapy. Her clinical work focuses on children, adolescents, families, trauma, ADHD, reunification, and play therapy, with training in modalities including EMDR, Somatic Experiencing, CBT, behavioral interventions, and expressive therapies. She is also the founder of Auxilium, a nonprofit organization dedicated to expanding access to mental health education, resources, and support. Dr. Rogers-Larke is passionate about equipping counselors and educators with practical, ethical, and clinically meaningful tools that can be applied directly to their work.
